Recognizing Pelvic Limb Lameness in Your Dog: How to Tell Which Back Leg Is Hurting

One day your dog is running across the yard.
The next, something just doesn't look right.
Maybe they're limping. Maybe they're "bunny hopping." Maybe they're reluctant to jump into the car. Or perhaps they simply seem slower than they used to be.
Unlike front leg lameness, which often causes obvious head bobbing, hind limb (pelvic limb) lameness can be much more subtle.
Understanding what to look for can help you recognize orthopedic problems earlier and get your dog the treatment they need.
What Is Pelvic Limb Lameness?
Pelvic limb lameness simply means your dog is painful or unable to use one (or both) hind legs normally.
The source of pain can originate from:
Hip
Stifle (knee)
Hock (ankle)
Paw or toes
Muscles
Tendons
Ligaments
Bones
Spine
Nerves
The challenge is that many of these conditions produce similar-looking gaits.
The Goal of Gait Analysis
When veterinarians watch a dog walk or trot, we aren't simply confirming that the dog is lame.
We're asking several questions:
Which leg is painful?
During which phase of the stride does the lameness occur?
Is the problem orthopedic or neurologic?
Is the pain located in a joint, muscle, tendon, or bone?
Is one limb affected—or both?
Every step provides valuable diagnostic information.
Unlike Front Limb Lameness...
Many owners learn the phrase:
"Down on sound. Up on lame."
That's an excellent rule for front leg lameness.
Unfortunately...
There is no equally reliable head-bobbing rule for hind limb lameness.
Instead, veterinarians focus on what the pelvis is doing.
Watch the Hips, Not the Head
One of the most useful things you can do is stop watching your dog's head.
Instead...
Watch the hips.
When viewed from behind during a trot, subtle differences become much easier to appreciate.
Hip Hike (Pelvic Rise)
One of the most important signs of hind limb pain is called a hip hike.
As the painful leg bears weight, the dog lifts that side of the pelvis higher than normal.
Why?
Because lifting the hip reduces the amount of weight placed on the painful limb.
Think of it as the hind limb equivalent of a front limb head bob.
If the right hip rises higher, the right hind leg is often the painful limb.
Shortened Stride Length
Dogs with hind limb pain often take a shorter step with the affected leg.
This is especially common with:
Cranial cruciate ligament disease
Hip osteoarthritis
Iliopsoas injury
Stifle pain
Rather than fully extending the limb, they quickly place it back on the ground to minimize discomfort.
Reduced Push-Off
Another clue is decreased propulsion.
Watch your dog from behind.
Does one leg seem to push off less forcefully?
Healthy dogs generate nearly equal propulsion from both hind limbs.
Dogs with pain frequently "cheat" by relying more heavily on the opposite leg.
Toe-Touching
Some dogs barely place the affected foot on the ground.
Instead, they'll:
Lightly touch the toes
Immediately lift the foot
Shift weight elsewhere
Toe-touching usually indicates significant discomfort.
Bunny Hopping
Owners often notice both hind legs moving together.
This is called bunny hopping.
It frequently occurs with:
Hip dysplasia
Bilateral hip arthritis
Bilateral cruciate disease
Lumbosacral disease
Rather than alternating normally, both legs move simultaneously because it's more comfortable.
Sitting Abnormally
Healthy dogs usually sit squarely.
Dogs with pelvic limb pain may:
Sit with one leg kicked out
Lean to one side
Avoid fully flexing one knee
Constantly reposition themselves
Although subtle, this can be an early sign of orthopedic disease.
Difficulty Standing
Owners often describe:
"Once he's up, he walks okay."
Difficulty rising commonly occurs with:
Hip arthritis
Cranial cruciate ligament disease
Muscle weakness
Neurologic disease
The transition from sitting to standing requires considerable strength and joint stability.
Difficulty Jumping
One of the earliest signs of hind limb pain is avoiding activities the dog previously enjoyed.
Examples include:
Jumping into the car
Climbing stairs
Jumping onto furniture
Playing fetch
Many owners mistakenly assume their dog is "just getting older."
Often, they're actually painful.
Muscle Loss
Chronic lameness leads to muscle atrophy.
Compare both thighs.
Does one side appear smaller?
Muscle loss is one of the strongest indicators that a limb has been painful for some time.
Common Causes of Pelvic Limb Lameness
Cranial Cruciate Ligament Disease
The most common orthopedic disease affecting the canine hind limb.
Typical signs:
Sudden lameness
Toe-touching
Difficulty rising
Sitting abnormally
Muscle loss
Hip Dysplasia
Often begins during puppyhood but becomes more obvious with age.
Signs include:
Bunny hopping
Reluctance to jump
Difficulty standing
Stiffness after exercise
Osteoarthritis
Arthritis can affect:
Hips
Knees
Hocks
Toes
Signs often develop gradually over months or years.
Luxating Patella
Common in small breeds.
Owners often describe:
"He skips a few steps... then walks normally again."
Iliopsoas Injury
Frequently affects athletic dogs.
Pain is often worse:
After running
During stretching
While accelerating
Neurologic Disease
Not every hind limb problem is orthopedic.
Neurologic disorders can produce:
Knuckling
Scuffing nails
Crossing limbs
Weakness
Incoordination
This is why a neurologic examination is often performed alongside the orthopedic examination.
Why Veterinarians Prefer the Trot
Walking can hide subtle lameness.
Trotting exaggerates asymmetries because:
Forces increase
The gait becomes more symmetrical
Compensation becomes easier to identify
Whenever possible, veterinarians evaluate both the walk and the trot.
Recording Videos at Home
One of the most helpful things you can do before your appointment is record your dog.
Try to capture:
✅ Walking away from the camera
✅ Walking toward the camera
✅ Trotting away
✅ Trotting toward
✅ Side views
Use a flat, non-slip surface and avoid retractable leashes.
Slow-motion video is especially valuable.
Many subtle abnormalities become obvious only when viewed frame by frame.
What You Shouldn't Do
Don't try to diagnose the problem based on gait alone.
Many different diseases produce similar movement patterns.
For example:
A dog with hip dysplasia may appear similar to one with a cruciate ligament tear.
Likewise, arthritis, muscle injuries, tendon injuries, and neurologic diseases can all cause comparable gait changes.
Gait analysis helps localize the problem—but rarely provides the complete diagnosis.
When Should You See Your Veterinarian?
Schedule an examination if your dog:
Limps for more than 24–48 hours
Refuses to bear weight
Cries when walking
Has swelling around a joint
Stops exercising
Has difficulty rising
Frequently slips or falls
Develops muscle loss
Early diagnosis often leads to simpler treatment and better long-term outcomes.
The Bottom Line
Recognizing pelvic limb lameness isn't as straightforward as spotting a front limb head bob, but there are several clues that can help. Watching your dog's hips instead of their head, looking for a hip hike, shortened stride, reduced push-off, toe-touching, bunny hopping, or difficulty rising can reveal that something is wrong long before the problem becomes severe.
While these observations can help you recognize that your dog may be painful, they are only the first step. A complete orthopedic and neurologic examination—often combined with imaging such as X-rays, ultrasound, CT, or MRI—is needed to determine the exact cause.
The earlier hind limb lameness is recognized, the more treatment options are available, helping your dog stay active, comfortable, and mobile for years to come.
